As a member of the Mary Berglund Community Health Centre Hub team, the successful candidate will be a Social Worker who is registered with the College of Social Workers of Ontario. The position is to provide clinical service to clients. This includes assessment, crisis intervention, therapeutic modalities based on needs, advocacy and referrals/linkages to community supports, education and coordinated discharge planning and follow up.
